Case #37: Woman in her 70s from Setagaya (hyperopic eyes, trifocal multifocal lens: PanOptix)

This is an English translation of the original Japanese case report. If the two differ, the Japanese version takes precedence. Visual acuity is shown in decimal notation (1.0 ≈ 20/20).

Change in visual acuity (decimal; corrected values in parentheses)

Right eye: pre-op → post-op
Far
0.3(0.4)→1.2(n.c.)
Near
—→1.5(n.c.)
Left eye: pre-op → post-op
Far
0.3(0.4)→0.9(1.0)
Near

A friend who works in insurance and had had surgery at our clinic recommended us, and she first visited wishing for cataract surgery with multifocal lenses. Since the previous year she had complained that both far and near were hard to see; besides age-appropriate cataracts she had also become hyperopic, and vision had fallen to about 0.3–0.4 in both eyes even with correction.
She did not drive and wished for surgery with PanOptix, the trifocal lens covered by advanced medical care; she wanted to prioritize near office work, so we set the non-dominant eye's target refraction slightly toward near.
Her eyes were short, but the surgery was completed without problems and without postoperative refractive error, and she was very satisfied with binocular uncorrected vision of 1.5 at distance and 1.2 at near. PanOptix's near focal distance is about 40 cm, so 30 cm may be somewhat harder to see. Petite people like her often look at things somewhat closer than the usual reading distance, so we recommend establishing your desired distance properly before surgery.

What was your condition before surgery?

Fine details were hard to see.
The sun was dazzling. Reading was hard.

What led you to have surgery?

Because everyone I knew who had the surgery was satisfied with the result.

Which did you choose, a multifocal lens or a monofocal lens?

I learned that advanced medical care insurance would cover it, so I chose multifocal lenses.
