Cataract Surgery Cases / Case

Monofocal IOL

Case #8: Man in his 70s from Suginami (mature cataract: monofocal lenses)

This is an English translation of the original Japanese case report. If the two differ, the Japanese version takes precedence. Visual acuity is shown in decimal notation (1.0 ≈ 20/20).

Change in visual acuity (decimal; corrected values in parentheses)

Right eye: pre-op → post-op
Right: counting fingers (not measurable)Right: 1.2(1.5)
Left eye: pre-op → post-op
Left: 0.1(0.8)Left: 1.2(1.5)

It was a mature cataract left untreated for many years, and the fundus could not be seen, so an accurate fundus assessment was impossible; but vision, counting fingers (just able to tell how many fingers are held in front of the eye) before surgery, improved to 1.2 after surgery, and he was very satisfied.

It was a mature cataract in one eye only, and he had surgery understanding the risk that, with zonules weakened by trauma, the intraocular lens might not go into the capsular bag. In the past this case would have been treated with an incision of about 8.0 mm and extracapsular cataract extraction (ECCE), but by staining the anterior capsule and using the latest balanced tip with efficient ultrasound, we completed the surgery as planned through a 2.4 mm incision, without complications such as corneal endothelial damage.

We will continue to actively adopt new techniques and strive to reduce the burden on patients.

Everyone warned me about it.

Which did you choose, a multifocal lens or a monofocal lens?

On the doctor's recommendation, I chose monofocal lenses myself.
