Multifocal IOL

Case #31: Woman in her 50s from Asaka, single-eye surgery (after LASIK, extended depth of focus multifocal lens: TECNIS Symfony)

This is an English translation of the original Japanese case report. If the two differ, the Japanese version takes precedence. Visual acuity is shown in decimal notation (1.0 ≈ 20/20).

She had a cataract in one eye after LASIK and first visited us wishing for cataract surgery with a multifocal lens.
Because LASIK removes corneal tissue, standard intraocular lens power formulas produce refractive error. So, in addition to special formulas recommended by the American Society of Cataract and Refractive Surgery, as with the patient in Cataract Surgery Case #14, we used the average corneal power from the OPD-Scan III®, a wavefront analyzer, and determined the lens power with the Camellin-Calossi formula. As for the target refraction, after discussion with her we set TECNIS Symfony slightly near-dominant.
She had also brought her pre-LASIK data, so we could achieve the target refraction with greater accuracy, and she was very satisfied with good uncorrected vision of 1.0 or better at both distance and near. Recently several formulas that do not use pre-LASIK data have been developed, but in my personal experience, formulas using preoperative data still allow more accurate lens power selection.
To get a more satisfying result, if you have had LASIK, we recommend obtaining your pre-LASIK data, or, if that is impossible, at least keeping a record of the glasses or contact lenses you used before.

Patient questionnaire

Answers translated from Japanese.

What was your condition before surgery?

My left eye was hazy and I could hardly see.

What led you to have surgery?

I had tests at a nearby eye clinic, but when I asked the director questions, his attitude was dismissive; searching the web, including your website, it seemed you would answer my questions properly, so I came.

Which did you choose, a multifocal lens or a monofocal lens?

I wanted to see both near and far reasonably well without glasses, so I chose a multifocal lens.
