He first visited us with reduced vision from a nuclear cataract that had progressed after vitreous surgery. He had always been highly myopic in both eyes, and at a well-known eye clinic in Tokyo he had had vitreous surgery for epiretinal membranes in both eyes and retinal detachment in the left eye; they had also recommended cataract surgery there, but he came to us wishing for surgery with MiniWELL.
The left eye had mild macular retinal atrophy after surgery, but the right eye had no macular abnormality and a more advanced cataract, and he complained strongly of reduced vision, so we first planned surgery on the right eye only.
He had studied multifocal lenses considerably in advance and struck me as very particular about quality of vision, so we judged MiniWELL, with almost no halos or glare, suitable; he wanted to prioritize desk work such as computers for his job, so we set the target slightly near-dominant.
After vitreous surgery the eye is vitrectomized, so there could be more risk than in ordinary cataract surgery, but the surgery was completed without problems, and after surgery he was satisfied with good uncorrected vision at both distances, 1.2 distance / 1.5 near, and especially delighted that near was clearer than expected.
MiniWELL's published near add is +3.0 D, but one often sees reports of weak near vision, and he too was worried about this. At our clinic, however, by setting the postoperative target refraction with reference to the preoperative refraction and the refraction with the contact lenses in use, we have so far had no cases of dissatisfaction with near vision, so if you are considering it, please feel free to consult us.

Patient questionnaire

Answers translated from Japanese.

What was your condition before surgery?

I couldn't focus, and even after changing my glasses' prescription things soon became hard to see again.

What led you to have surgery?

Because I learned online that there's no way to improve it other than surgery.

Which did you choose, a multifocal lens or a monofocal lens?

I chose multifocal, because I wanted to reduce the times I need glasses.
